The study, published in the journal Cell, focuses on nitric oxide, a compound that dilates blood vessels, improves memory, fights infection and stimulates the release of hormones, among other functions.

How nitric oxide performs these activities had long been a mystery.

The researchers discovered a novel “carrier” enzyme (called SNO-CoA-assisted nitrosylase, or SCAN) that attaches nitric oxide to proteins, including the receptor for insulin action.

They found that the SCAN enzyme was essential for normal insulin action, but also discovered heightened SCAN activity in diabetic patients and mice with diabetes.

Mouse models without the SCAN enzyme appeared to be shielded from diabetes, suggesting that too much nitric oxide on proteins may be a cause of such diseases.

“We show that blocking this enzyme protects from diabetes, but the implications extend to many diseases likely caused by novel enzymes that add nitric oxide,” said lead researcher Jonathan Stamler from the Case Western Reserve School of Medicine. “Blocking this enzyme may offer a new treatment.”

Given the discovery, next steps could be to develop medications against the enzyme.

Excessive nitric oxide has been implicated in many diseases, but the ability to treat has been limited because the molecule is reactive and can’t be targeted specifically.

Having ascended to the pinnacle of medical achievements, Dr Jain is solidifying his legacy by setting more and more world records to his credit.

Dr Amit Kumar C Jain, an expert Diabetic foot Surgeon (podiatric surgeon), over years, has already garnered numerous monikers for his phenomenal work in the diabetic foot, such as “The Doc who Rocked”, “The Father of Diabetic Foot Surgery”, “The Rocking Doc”, “This Doc continues to Rock”, etc.

In one of the remarkable feat, Dr Amit Jain has become the youngest Indian medical professional at 39 years old to earn the illustrious title of ‘Father’ within the domain of Diabetic Foot, a distinction etched into the annals of authoritative medical literature for his innovative work. This extraordinary accomplishment is substantiated by years of pioneering work in diabetic foot published in literature, backed by comprehensive evidence of records found in reputable sources like Best of India’s Record, Indian Book of Records, India’s World Records, Kalam’s World Records, and recently in the World Records India 2023.

Dr. Amit Jain has previously held the honour of being the youngest Indian doctor to articulate foundational principles and innovative practices within the realm of diabetic foot field. Centuries ago, in Indian medical history, Sushruta had established fundamental principles and practices and in today’s era, Dr Amit Jain, who represents the new generation of doctors, carries forward the legacy of laying down the principle and practice.

Dr. Jain’s legacy had earlier comprised of six world records, a testament to his unwavering dedication and outstanding achievements as an Indian surgeon. Recently, Dr Jain has added a seventh world record to his armamentarium of records. He holds the distinction of having the most number of named work published in a book and this record is set in Best of India’s record 2023.

In the tapestry of medical advancements, very few doctors manage to ascend to the realm of true originality. Dr. Amit Jain stands as a remarkable exception, showcasing a legacy of meticulously planned contributions and unyielding dedication to pushing the boundaries of the field.

Dr. Amit Jain’s impact resonates far beyond India’s borders, casting a global shadow. His revolutionary principles and pioneering practices in diabetic foot care have reshaped the field, earning him the prestigious distinction of “Father of Diabetic Foot Surgery” years ago that is etched in Indian Book of Records, Kalam’s World Record, Best of India’s Records and World Record India. Applauded by Dr. Gopal S, an esteemed Professor of Surgery at Dr. Chandramma Dayananda Sagar Institute of Medical Education and Research, Bengaluru, Dr. Jain’s role in propelling diabetic foot surgery (Podiatry) to new heights is nothing short of transformative, setting new benchmarks in innovation and excellence.

In recent times, there has been a propensity to label doctors as “Legends” or “Fathers” either with no reasons or for absurd reasons by different agencies, pharmaceutical industries, or doctor’s associations without any of them having any substantial original innovative work in medical literature to support these titles. Dr. Gopal emphasizes that such titles should be reserved for true pioneers of the subject like Dr. Amit Jain, who exhibit a consistent history of exceptional, persistent, original, and innovative work of distinction and excellence. Identifying such trailblazers often requires delving deep into the annals of medical literature by research experts or academicians, states Professor Gopal, who had reviewed the modern diabetic foot surgery system from India.

The impact of Dr. Amit Jain’s pioneering work reaches far beyond recognition within medical literature. Brindhavvan Areion Hospital’s leadership has immortalized his contributions by dedicating a wing in his honour, an accolade that has been enshrined in the Grand Asian Book of World Records in 2022. This gesture pays homage to his outstanding research, surgical expertise, educational endeavours, and philanthropic pursuits.

Dr. Kishore Kumar, one of the Directors of the hospital, underscores the global resonance of Dr. Amit Jain’s contributions to diabetic foot/Podiatry. His unique teaching tools, methodologies and captivating original innovative presentations of his work have also earned accolades and his teaching tools being chronicled in Best of India’s Records.

It occurs when high blood sugar levels damage the blood vessels in the retina, leading to vision impairment and, in severe cases, even blindness. The question that many individuals with diabetes have is whether diabetic retinopathy can be reversed. In this blog, we will explore the different subtopics related to this question and shed light on the possibilities of reversing the effects of diabetic retinopathy.

Diabetic Retinopathy Eye Problems Treatment Options:

Early Detection and Management

Early detection plays a crucial role in managing diabetic retinopathy. Regular eye examinations are essential for monitoring the condition and identifying any changes in the eyes. With advancements in medical technology, eye specialists can now detect diabetic retinopathy at its early stage, providing an opportunity for early intervention and management. By keeping blood sugar levels under control, managing blood pressure, and adopting a healthy lifestyle, individuals can slow down the progression of the condition and minimize further damage to the retina.

Laser Treatment and Surgical Interventions

In certain cases, laser treatment or surgical interventions may be recommended to address diabetic retinopathy. Laser procedures, such as focal laser treatment or scatter laser treatment, aim to seal leaking blood vessels and prevent further vision loss. In more advanced cases, surgical procedures like vitrectomy may be considered, where the surgeon removes the vitreous gel and replaces it with a saline solution to improve vision. These treatments can help to stabilize the condition, prevent further damage, and, in some cases, improve vision to some extent.

Anti-VEGF Injections

Anti-VEGF (vascular endothelial growth factor) injections have shown promising results in reversing the effects of diabetic retinopathy. Anti-VEGF injections work by inhibiting the action of this protein, reducing the growth of new blood vessels and minimizing leakage. Studies have demonstrated that these injections can improve vision and prevent further vision loss in individuals with diabetic retinopathy. However, it is important to note that the effectiveness of these injections may vary from person to person, and multiple injections may be required over a period of time.

Lifestyle Changes and Co-management

While medical interventions are important, lifestyle changes and co-management play a significant role in reversing or slowing down the progression of diabetic retinopathy. A healthy lifestyle that includes regular exercise, a balanced diet, maintaining a healthy weight, and avoiding smoking can have a positive impact on eye health. Additionally, working closely with healthcare professionals, including ophthalmologists, endocrinologists, and diabetes educators, helps ensure effective management of diabetes and its associated complications, such as diabetic retinopathy.

While complete reversal of diabetic retinopathy may not always be possible, early detection, effective management, and advancements in treatment options offer hope for individuals with the condition. Through a combination of medical interventions, lifestyle changes, and co-management, the progression of diabetic retinopathy can be slowed down, vision loss can be minimized, and in some cases, improvements in vision may be achieved. It is crucial for individuals with diabetes to prioritize regular eye examinations and work closely with healthcare professionals to ensure optimal eye health and overall diabetes management.

Author:

Diabetes mellitus is a widespread chronic condition that affects millions of people worldwide. Among the affected populations, young Asian Indian adults face a growing concern due to the high prevalence of Type 2 Diabetes Mellitus (T2DM). However, recent research has shed light on a promising approach to tackle this epidemic - Intensive Lifestyle Therapy.

Type 2 Diabetes Mellitus is a metabolic disorder characterised by elevated blood sugar levels due to insulin resistance or insufficient insulin production. Asian Indian young adults are particularly susceptible to developing this condition due to genetic predisposition, lifestyle factors, and dietary habits.

The core of intensive lifestyle therapy revolves around two essential components - emphasising diet and promoting physical activity.

Diet plays a crucial role in diabetes management and reversal. A focus on whole, unprocessed foods, low in refined sugars and carbohydrates, can help regulate blood glucose levels and promote weight loss. Physical activity has been proven to enhance insulin sensitivity and overall metabolic health. Engaging in regular exercise can aid in diabetes management and contribute to achieving reversal.

Numerous studies have investigated the impact of intensive lifestyle therapy on diabetes reversal among Asian Indian young adults. These studies have reported impressive findings, indicating a potential breakthrough in diabetes management. Researchers have conducted controlled trials and longitudinal studies to assess the effects of lifestyle intervention on diabetes outcomes. The results have shown promising reductions in HbA1c levels and improved insulin sensitivity . The data from these studies have highlighted that a significant proportion of newly diagnosed Asian Indian young adults with T2DM were able to achieve diabetes reversal through intensive lifestyle therapy.

Intensive lifestyle therapy presents a promising and transformative approach to diabetes management, especially among newly diagnosed Asian Indian young adults with Type 2 Diabetes Mellitus. The potential for diabetes reversal through targeted lifestyle changes offers hope in combating the Asian Indian diabetes epidemic and improving the overall health and well-being of affected individuals and communities. As research in this field progresses, there is the promise of a brighter future in the fight against diabetes.

Dr. Ramagiri’s method merges the latest lifestyle medicine research with contemporary medical practices. It enables individuals to reverse their conditions through sustainable lifestyle changes. His Health Easy app, which facilitates online consultations and health monitoring, ensures patients have the tools and support they need for lasting health transformations.

Diabetic retinopathy is a serious sight-threatening complication of diabetes and can cause permanent vision loss and blindness.

The study led by a team from the Michigan State University in the US showed that crystalised deposits are very reflective and can be seen in images of the retina. This is important because noninvasive retina evaluations can be done by most optometrists, creating an opportunity for earlier diagnosis for more people, they wrote in the paper published in the journal Diabetologia.

"Retinopathy is the leading cause of preventable blindness and one of the most feared complications of Type 1 and Type 2 diabetes," said Julia Busik, MSU professor emeritus of physiology.

"Within 20 years of developing diabetes, every individual with either Type 1 or Type 2 diabetes will have some degree of retinopathy. Current treatment approaches are very invasive and are only directed at the very late stage of retinopathy."

In the study, the researchers discovered that these cholesterol crystals are like the crystals found in atherosclerotic plaque that can form in arteries and cause heart attacks,

George Abela, chief of the MSU Division of Cardiology, helped the team identify ways to scan retinas using modified tissue preparation for scanning electron microscopy. This also helps researchers analyse the composition of the crystals, which typically result when there is too much cholesterol in one place.

"We are actively pursuing what can be done to lower cholesterol in the retina," said Tim Dorweiler, a doctoral candidate in the Molecular, Cellular and Integrated Physiology Program at MSU and first author of the paper.

"The retina is a very isolated organ, just like the brain, and both have a blood barrier that separates them from the rest of the body. This is what makes the retina hard to study and extremely complex."

There is also hope that new treatments to address crystals formed by cholesterol could be less invasive than current options for diabetic retinopathy. And there are questions about other areas of the body where these crystals could be treated to prevent other diseases.

Health shouldn’t hold anyone back professionally. One of the most growing health conditions today that people need to manage effectively – including in the workplace - is diabetes. With careful planning, you can keep your blood sugar levels in check,so you continue to be at your best – throughout the work day and beyond.

Dr. Ravi Sankar Erukulapati, Endocrinologist, Apollo Hospitals, Hyderabad, commented, “India now has over 101 million people living with diabetes – a rise from 77 million in 2019. Diabetes is a chronic lifestyle condition that needs to be managed properly to lessen the risk of other health problems in the future. For working professionals, many of whom are doing desk jobs, this can be challenging. As a result, people living with diabetes are sometimes inconsistent with their lifestyle changes and don’t take their medicines as prescribed. It is critical for people with diabetes to take necessary steps to effectively manage their diabetes and properly follow the treatment schedule for better health outcomes.”

Here are 7 steps on how to manage diabetes effectively at work:

Have a diabetes action plan that suits your worklife: Navigating your diabetes and work journey starts even before you reach the office. Getting good sleep at night matters – as does how you plan for the day. Create a routine so you don’t skip breakfast – which is important to keep sugar highs and lows at bay.

Decide whether you want to have this meal at home or when you get to work but keep it nutritious. Limit empty calories and cut back on salt and saturated fat.

Snack smart: Whether a co-worker is munching on chips or fried foods or candy, or offering everyone mithai with celebratory news, you are always surrounded with unhealthy temptations.Avoiding snacking entirely can be tough, so have them in moderation, and be mindful of what you eat. Keep healthy snacks at hand – fruits, salad, nuts, yogurt – for when cravings hit. Also remember to stay hydrated with water over sugary and caffeinated drinks.

Healthy Lunch Matters: Pack lunch smartly so you can enjoy a healthy, balanced meal and aren’t only dependant on outside food. A diabetes-friendly diet includes leafy greens (like spinach), non-starchy vegetables (like carrots, tomatoes, onions, okra, cauliflower), healthy carbs (like whole grains and brown rice), lean proteins (including eggs, beans, and chicken), and fruits low in carbohydrates (such as oranges). On days with special events, whether a colleague’s birthday or team lunch, watch your calorie intake beforehand.

In addition to following a balanced diet, it is crucial to monitor your glucose levels before and after meals. Continuous Glucose Monitoring (CGM) devices can be helpful in tracking your glucose levels throughout the day, providing real-time data to aid in managing your diabetes.

Stick to your medication routine: To manage your diabetes well, adhering to medication is important, which helps maintain glycemic control. In case you need to take any medication at work, be sure to set reminders – maybe on your phone or with a post-it note on your desk.

Ashwini Pawar, Medical Affairs Director for Abbott in India said, “For people with diabetes, disease management is important for maintaining good health. However, many don’t take their medicines as prescribed. At Abbott, we go beyond treating diseases by helping people build better habits and addressing typical barriers for taking timely medication. Introducing patient-centric innovations such as making tablets with a special coating that make them easier to swallow, can help people overcome difficulties. Through such measures, we hope to help patients effectively manage diabetes so they can lead healthier lives while performing at their best.”

Let your team be in the know: If you feel comfortable, talk to your manager and team about living with diabetes. This may help you request for more flexible adjustments, or even consistency with lunch breaks. You can also discuss critical steps necessary in case of a glucose emergency, so they know how to help you if such situations arise.

Get moving: Many at work have sedentary lifestyles. Physical activity helps people with diabetes manage their condition better. Get active by stretching at your desk, taking short walks around the office, up and down the stairs, or even outside. A recent study found that a post-meal walk also helps lower your blood sugar levels. Further, try fitting exercise into your routine before or after work.

Learn to manage stress: When you are stressed, you may notice your glucose levels changing. Sometimes, you may be overwhelmed at work. Good coping mechanisms help keep your blood sugar levels in check when things get difficult. Try meditating in a quiet corner, find time to unwind (talk to a colleague or take a break), and identify and manage stressors.

Try finding a diabetes-friendly routine that works for you, so you can better manage your diabetes at work!

With the increasing prevalence of diabetes, it has become imperative to explore alternative treatment options that complement conventional medical interventions. India alone has seen a 44 per cent increase in diabetic cases over the last four years and about 100+ million people are affected by diabetes. Combined with a healthy lifestyle, proper nutrition, and regular exercise, Ayurvedic herbs can contribute to better blood sugar control and improve the overall quality of life for individuals living with diabetes.

Ayurveda, an ancient system of medicine originating from India, offers a holistic approach to managing diabetes. Ayurvedic herbs have gained recognition for their potential for blood sugar regulation and can serve as valuable adjuncts to conventional therapies. Here is a list of Ayurvedic herbs for the natural management of diabetes.

Karela (Bitter Melon):

Karela, also known as Bitter Melon, has long been used in Ayurveda for its anti-diabetic properties. It contains an insulin-like compound called polypeptide-p, which helps lower blood sugar levels. Bitter melon also aids in improving glucose utilisation and stimulating insulin secretion, making it an excellent choice for diabetes management.

Jamun:

Jamun, also known as Indian blackberry or black plum, has hypoglycemic effects, aiding in lowering blood sugar levels. It contains bioactive compounds like anthocyanins, ellagic acid, and polyphenols, which further aid the process. Consumption of jamun or its juice helps regulate blood glucose levels, improve insulin sensitivity, and reduce the risk of diabetic complications. Its high dietary fibre content slows down sugar absorption, promoting better glycemic control. Including jamun in a balanced diet can effectively manage diabetes.

Giloy (Tinospora Cordifolia):

Giloy, also known as Tinospora Cordifolia, exhibits hypoglycemic effects by stimulating insulin secretion and improving insulin sensitivity, helping to regulate blood sugar levels. Additionally, its anti-inflammatory properties can reduce inflammation and enhance insulin sensitivity. Giloy's antioxidant property helps protect pancreatic beta cells and counteract the oxidative stress associated with diabetes. Incorporating Giloy into a comprehensive diabetes management plan may provide additional support for maintaining stable blood sugar levels and promoting overall well-being.

Gudmar (Gymnema Sylvestre):

Gudmar, also known as Gymnema Sylvestre, has been recognised for its potential role in managing diabetes naturally. It may help control blood sugar levels by reducing glucose absorption and enhancing insulin sensitivity. Additionally, Gudmar has been associated with managing sugar cravings and supporting pancreatic health.

Indian Gooseberry (Amla):

Indian Gooseberry, or Amla, is a potent Ayurvedic herb that offers numerous health benefits, including blood sugar regulation. It is rich in vitamin C and antioxidants, which help improve pancreatic function and enhance insulin secretion. Amla also aids in reducing oxidative stress and inflammation, which are often associated with diabetes.

While Ayurvedic herbs cannot replace conventional medical treatments for diabetes, they can play a significant role in managing blood sugar regulation and overall diabetes naturally. The herbs mentioned above offer a range of potential benefits, from improving insulin sensitivity to reducing oxidative stress.

Light intensity walking for 3 minutes every 30 minutes can help people with Type 1 diabetes to manage their blood sugar levels, according to a study.

Type 1 diabetes is a chronic condition in which the pancreas produces little or no insulin.

Taking short breaks from sitting time -- also known as 'activity snacking' -- is a simple, cost-free way to lower average blood sugar levels, compared to uninterrupted sitting and also potentially reduce their risk of future complications.

"For people with type 1 diabetes, managing blood sugar levels day in and out is relentless. Being physically active is important in managing the condition, but building exercise into your daily routine can be challenging, and even those who exercise frequently can often spend a lot of time sitting or lying down," said Dr Elizabeth Robertson, our Director of Research, from Diabetes UK.

Robertson suggested making a simple, practical change -- such as taking phone calls while walking, or setting a timer to remind you to take breaks -- to avoid sitting for long periods.

Previous research has shown that breaking up periods of sitting with short, frequent walks can help people with Type 2 diabetes reduce their blood sugar levels and their risk of complications. This is because being active can increase the amount of glucose (sugar) used by muscles and can help the body to use insulin more effectively.

But until now it was not known if people with Type 1 diabetes could see the same benefits.

To understand, the team recruited 32 participants, who were, during one session, asked to remain seated for a full seven hours. During the other, they broke up their sitting time with three-minute bouts of light intensity walking every 30 minutes.

Participants wore a continuous glucose monitor (CGM) to track their blood sugar levels for a 48-hour period during and after each sitting session.

Regular walking breaks resulted in lower average blood sugar levels. Importantly, the breaks from sitting also did not increase low blood sugar levels -- a common occurrence with more traditional types of physical activity and exercise.

"Breaking up prolonged sitting with light-intensity activity is something that people can do irrespective of whether they currently exercise or not. For some people, 'activity snacking' could be an important stepping-stone towards more regular physical activity or exercise, whereas for others, it may be a simple and acceptable intervention to help manage blood glucose levels," said Dr Matthew Campbell, at the University of Sunderland.
